Medical Consent for U18s

I consent to my childs medical details to be shared with coaches/leaders for the purposes of the delivery of my child’s safe participation in the cricket club activity. Not providing consent will not affect your child’s membership to the Club, however giving us consent to share this information will help club volunteers to know how to respond effectively in the case of any medical emergency.

PARENT/LEGAL GUARDIAN PARTICIPATION AGREEMENT

I have read Belvoir Cricket Club Code of Conduct for members and guests and my child has read the BCC Junior Code of Conduct

I have read, or have been made aware of, the following club policies

Supervision of children (including section on transporting children and lost child policy)

Social media, texts, email, photography and filming policy (including section on communicating with U18 players)

Changing room and showering policy

Anti-bullying policy

Junior players in open-age (senior) cricket policy

I understand and agree to the responsibilities which I and my child have regarding these policies

I confirm I have joined the relevant club What’s App group to be informed of my child’s fixture details.

I consent to my child playing in away fixtures and events

For children who meet the minimum age requirement* to play in senior cricket

I agree to my child playing open-age (senior) cricket * The minimum age is the U13 age group. (Players must be in Year 8, and 12yrs old on 1 September of the preceding year.)

I consent to the club photographing or videoing my childs involvement in cricket in line with the club photography/video policy.

I consent to the club publishing photos of my child in line with the club photography/video policy (If you do not wish to give consent for this please contact us to discuss how we can manage any potential photography or filming. Not giving consent will not affect your child’s membership of the club)
